ABSTRACT

Exposure to hypoxia is considered a stressful stimulus, therefore the organism develops acclimation mechanisms to try to ensure homeostasis. Although the effect of hypoxia on different tissues and organs has been very well documented, the role of low levels of O2 in the oral cavity has not received the same analysis. In this review, we analyzed the available bibliographic data concerning the effects of hypoxia on periodontal tissue, salivary glands, and dental pulp. The published evidence demonstrates that low O2 levels increase the number of inflammatory mediators that induce the progression of periodontal disease, and, in turn, the inflammation established during the progression of periodontitis aggravates tissue hypoxia conditions. Salivary glands are also affected during hypoxic exposure, decreasing salivary secretion, and leading to ultrastructural alterations in the glandular parenchyma. On the other hand, hypoxia could also be beneficial in some scenarios. It has been established that dental pulp cells grow better in culture under hypoxic conditions than they do in normoxia. Furthermore, mild hypoxia seems to stimulate periodontal ligament cells proliferation and matrix degradation, key events during for orthodontic treatments. In conclusion, to determine the effects of hypoxia in the oral cavity, it is necessary to analyze not only the type of tissue involved but also the hypoxic conditions to which it is subjected, as well as its duration and modality (AU)

ABSTRACT

Abstract To evaluate the impact of the GaAlAs diode laser with energy densities of 160 J/cm2, 320 J/cm2, and 640 J/cm2 on the periodontal tissues under continuous orthodontic force application and on the rate of orthodontic tooth movement in rats with type-2 diabetes mellitus. The intensity of primary alveolar bone formation was also investigated through the immune-positive osteocytes for OPN antibody. Forty adult male Wistar rats were divided into eight groups of 5 rats: normoglycemic (N), 160 J-laser-normoglycemic (160 J-LN), 320 J-laser-normoglycemic (320 J-LN), 640 J-laser-normoglycemic (640 J-LN), diabetic (D), 160 J-laser-diabetic (160 J-LD), 320 J-laser-diabetic (320 J-LD), and 640 J-laser-diabetic (640 J-LD) rats. Diabetes mellitus was induced by a single intravenous injection of 40 mg/kg monohydrated-alloxan. An orthodontic force magnitude of 20cN was applied. The laser parameters were continuous emission of 780-nm wavelength, output power of 20mW, and fiber probe with a spot size of 0.04 cm in diameter. Radiographic, histomorphological, and immunohistochemical analysis were performed after a period of 21 days. The photobiomodulation using the energy density of 640 J/cm2 strongly stimulated the alveolar bone formation and contributed the reorganization of the soft periodontal tissues, followed by the 320 J/cm2. Extensive alveolar bone loss, intense infiltration of inflammatory cells, and degradation of the PDJ tissue were mainly found in the D and 160 J-LD groups. The rate of orthodontic tooth movement was represented by the interdental distance between the cementoenamel junctions of the right mandibular first and second molars . This distance was larger in the diabetic groups (D: 39.98±1.97, 160 J-LD: 34.84±6.01, 320 J-LD: 29.82±1.73, and 640 J-LD: 35.47±4.56) than in the normoglycemic groups (N: 21.13±1.19; 160 J-LN: 22.69±0.72, 320 J-LN: 22.28±0.78, and 640 J-LN: 24.56±2.11). The number of osteopontin-positive osteocytes was significantly greater in the 640 J-LD (14.72 ± 0.82; p < 0.01) and 640 J-LN (13.62 ± 1.33; p < 0.05) groups than with D (9.82 ± 1.17) and 160 J-LD (9.77 ± 1.10) groups. Therefore, the energy density of 640 J/cm2 provided the best maintenance and integrity of the periodontal tissue microarchitecture under continuous orthodontic force when compared with the other dosages, mainly in the uncontrolled diabetic rats. The interdental distance was greater in the D and 160 J-LD groups due to presence of severe periodontitis caused by diabetes plus the mechanical stress generated by continuous orthodontic forces, implying, thus, an insufficient biostimulatory effect for the dosage of 160 J/cm2.

ABSTRACT

Until these days endodontic-periodontal lesions remains a challenge to dentists. A single lesion may show signs of endodontic and periodontal involvement, which can induce the dentist to perform an inadequate treatment. The diagnosis is often difficult due to the intimate anatomical and vascular connections between the pulp and periodontium. Furthermore, the fact that these diseases are often evaluated as different entities complicates the diagnosis. Prognosis assessment and treatment can also be challenging. Microorganisms, trauma, perforations and radicular resorptions are among the etiologic factors and contributors to the development and progression of the disease. Often the tooth affected by an endodontic-periodontal lesion is doomed due to lack of knowledge of the dentist. It is important to recognize the relationship between the diseases for the proper management of the injury. This study reviewed the literature related to the etiology, classification and treatment of this disease. The reviewwas done through consultation with pubmed and articles published between 1974 and 2016 were included. The terms "endo perio lesion", "endodontic periodontal lesion" and "endodontic periodontal disease" were the key words used in the survey. It was concluded that it must be addressed and treated in a multidisciplinary way. Treatment should be directed to the main cause. In true combine lesion, endodontic treatment should precede periodontal.(AU)

ABSTRACT

OBJECTIVE: The objective of this study was to assess the histological alterations that occurred in the periodontal area of rat molars submitted to induced tooth movement (ITM) right after an intentional trauma (subluxation). METHODS: Forty adult male Wistar rats (Rattus norvegicus albinus) were selected. The animals were divided into eight groups (n = 5), according to the combination of variables: Group 1 - control (neither trauma nor ITM); Group 2 - ITM; Groups 3, 4, 5 and 6 - dentoalveolar trauma groups corresponding, respectively, to 1, 3, 8 and 10 days after trauma; Groups 7 and 8 - the animals' molars were subjected to a 900 cN impact and, one and three days after the trauma event, tooth movement was induced. The rats' maxillary first molars were mesially moved during seven days with a closed coil (50 cN). After the experimental period of each group, the animals were sacrificed by anesthetic overdose and the right maxillas were removed and processed for histological analysis under light microscopy. RESULTS: In the animals of group 3, 4, 5 and 6, the histological alterations were not very significant. Consequently, the effect of induced tooth movement right after a subluxation event (groups 7 and 8) was very similar to those described for Group 2. CONCLUSION: There was no difference in the quality of periodontal repair when ITM was applied to teeth that had suffered a subluxation trauma. .

ABSTRACT

INTRODUCTION: Plaque control is the major consensus during orthodontic treatment to prevent the occurrence of cavities and periodontal inflammation. The mechanic resource of greater effectiveness and frequent use in this control is the oral hygiene. The tooth brushing techniques most used in orthodontic patients are: Ramfjord's method, Modified Stillman technique and Bass method. OBJECTIVE: Since control studies evaluating the effectiveness of usual tooth brushing techniques do not show clear advantage, the objective of this study was to evaluate the effectiveness of three brushing methods, through periodontal clinical parameters of patients with fixed orthodontic appliances. METHODS: Thirty patients were selected, with ages between 14 and 22 years old, with fixed orthodontic appliances. After basic periodontal treatment the following factors were evaluated: 1 - Plaque index and 2 - Gingival index and each patient was randomly included in one of the three selected groups according to the brushing technique: Group 1 - Scrubbing technique; Group 2 - Modified Stillman technique and Group 3 - Bass technique. Patients were evaluated for 9 months. RESULTS: The results showed a significant reduction of clinical parameters by the end of this period, however there was a very significant reduction of Gingival index on group 3 (13.6%) when compared to the other groups. CONCLUSION: Thus, it can be suggested that the Bass technique can be effective on the reduction of periodontal clinical parameters of Plaque index and Gingival index in patients with fixed orthodontic appliances.

ABSTRACT

Periodontal disease (PD) is characterized as an inflammatory process that compromises the support and protection of the periodontium. Patients with Down’s syndrome (DS) are prone to develop PD. Neutrophils (NE) are the first line of defense against infection and their absence sets the stage for disease. Aim: To compare the activity and function of NE in the peripheral blood from DS patients with and without PD, assisted at the Center for Dental Assistance to Patients with Special Needs affiliated with the School of Dentistry of Araçatuba, Brazil. Methods: Purified NE were collected from peripheral blood of 22 DS patients. NE were used to detect the 5-lypoxigenase (5-LO) expression by RT-PCR. Plasma from peripheral blood was collected to measure tumor necrosis factor-a (TNF-a) and interleukin-8 (IL-8) by ELISA and nitrite (NO3) using a Griess assay. Results: Data analysis demonstrated that DS patients with PD present high levels of TNF-a and IL-8 when compared with DS patients without PD. However, there was no statistically significant difference in the levels of NO3 production between the groups. The levels of the inflammatory mediator 5-LO expression increased in DS patients with PD. Conclusions: According with these results, it was concluded that TNF-a and IL-8 are produced by DS patients with PD. Furthermore, DS patients with PD presented high levels of 5-LO expression, suggesting the presence of leukotriene B4 (LTB4) in PD, thus demonstrating that the changes in NE function due to the elevation of inflammatory mediators contribute to PD.

ABSTRACT

The level of bone crest surrounding the implant is of utmost significance to determine osseointegrated implant success, as preservation of marginal bone height is highly important for long-term dental implant survival. Various approaches have been described in the literature to prevent the crestal bone loss, including platform switching, non-submerged approach, scalloped implants, implant design modifications, progressive loading, immediate implant placement, etc. The purpose of this paper is to review all the possible methods to preserve the crestal bone, when each method should be used and their success rates in an attempt to address this complex problem of crestal bone resorption. "PubMed" and "Google Scholar" were used to find out any studies involving platform switching concept from 1990 up to 2009. Twenty-four studies involving methods for preservation of crestal bone were evaluated, which included 26% studies on platform switching, 22% on non-submerged approach, 17% on scalloped implants, 13% on progressive loading and 22% on immediate implant placement. Crestal bone preservation should be thought of starting from the design of the implant to be placed. The technique to be followed in a given case will depend upon the density of bone, force factors by the patient, bone volume and amount of soft tissues, etc. The best possible method or the combination of the methods should be used to preserve the crestal bone for the long-term success of the implants.

ABSTRACT

Based on the latest researches it is concluded that periodontal disease is an important risk factor, leading to preterm delivery low birth weight babies and the effects of socioeconomic status on periodontal condition. The aim of this study, evaluation of periodontal condition of pregnant and non-pregnant women based on CPITN index and relation between the demographic and clinical variables. A cross-sectional descriptive study was carried out among 540 pregnant and non-pregnant women. Data collection was carried out with a questionnaire with demographic and clinical variables. Examinations were performed with WHO probes and dental mirrors. Data were analyzed by the means on Chi-square T-test and Anova. SPSS Software was used. The results of study showed that 66.7% pregnant women had periodontal disease and 92% pregnant women in third trimester had periodontal disease. There was a significant relation between CPITN and gestational age, education level, economic level and frequency of tooth brushing. The results of this study showed that pregnant women had higher CPITN than non pregnant women, then it might be suggested that simple preventive oral hygiene program help to maintain healthy periodontium during pregnancy

ABSTRACT

Internal root resorptions are usually non-symptomatic and are discovered occasionally throughperiapical radiographs, revealing a very defined and regular outline. These resorptions are progressive, and eventually perforate the periodontium. The present work describes the treatment of a clinicalcase of internal root resorption with periodontal communication, outlining the interrelation betweenperiodontal surgery and endodontic therapy.

ABSTRACT

ABSTRACT

The rat model is widely used in periodontal research and the quality of histological sections is essential. The purpose of this study was to evaluate the histological characteristics of periodontal tissues in Wistar rat maxillae, with different times of fixation and decalcified by nitric acid or formic acid (Anna Morse Solution). Fifteen rats were used. Fixation was performed for 24, 48 and 72 hours. The maxillae were hemi-sectioned and each part was decalcified either in nitric acid for 7 days or in Anna Morse solution for 35 days. Two trained and blinded examiners performed the evaluation. Fourty eight hours of fixation and decalcification with Anna Morse solution showed more clear characteristics of the epithelium-connective tissue interface and of the periodontal structures. Mean measurements between the cementum-enamel junction and the bone crest varied in the different experimental times from 176.5 (± 60.45) to 210.94 (± 39.33) pixels on the buccal aspect, and from 199.69 (± 38.33) to 298.55 (± 70.81) pixels on the palatal aspect, with no statistically significant differences (ANOVA, p > 0.05). In the same fixation period, decalcification with nitric acid or Anna Morse solution did not display any statistically significant differences. It may be concluded that for a qualitative histological analysis, fixation should preferably be for 48 hours and the demineralization should be made by Anna Morse solution. For a histomorphometric analysis, the decalcification solution does not interfere in the results.

ABSTRACT

BACKGROUND & OBJECTIVE: The role of lathyrogens on bone metabolism is unclear, therefore we undertook this study to observe periodontal and systemic alterations in experimental lathyrism in rat and compare these changes to that observed in the locally induced periodontitis group. METHODS: A total of 45 male Wistar rats were equally divided in the lathyritic group (group 1), ligature-induced periodontitis group (group 2), and healthy controls (group 3). Experimental lathyrism was induced by once daily subcutaneous administration of beta-aminoproprionitrile (beta-APN), at a dose of 5 mg/0.4 ml per 100 g of body weight for 40 days. Ligature-induced periodontitis was created by tying silk ligatures on the necks of mandibular molars. After 40 days, blood samples were obtained and the animals were decapitated. Radiographic observations, extraction tests, histologic evaluations were performed, and serum ALP activity and gingival tissue IL-1beta levels were measured. RESULTS: Significant alveolar bone resorption around the mandibular molar teeth (P<0.001); lower extraction force levels (P<0.001); higher numbers of lymphocytes and macrophages (P<0.01) (both in connective tissue and epithelium at the dentogingival junction); decreased ALP activity (P<0.001); and increased gingival tissue IL-1beta levels (P<0.001) were observed in groups 1 and 2, compared to those in group 3. ALP activity was higher in group 1 than in group 2 rats (P<0.05). INTERPRETATION & CONCLUSION: Similar radiographical and histopathological findings and comparable increases in gingival tissue IL-1beta levels both in groups 1 and 2 showed that in addition to resorption of alveolar bone, chronic inflammation of periodontium also occurred both in the lathyritic rats as well as in ligature-induced periodontitis group rats.

ABSTRACT

ABSTRACT

Recent researchs suggest that periodontophatogens microrganisms are transmitted among the members of the same family, because it could be observed the likeness of the lineages of obtained from bacterias of family members, mainly of the mother. The purpose of this study was to verify if there was some significant correlation between the results of the Test BANA (Perioscan), obtained in samples of subgingival dental plaque of 54 children between 6 and 9 years and their mothers, resident in the municipal district BarretosS.P. The samples of the subgingival plaques were picked up with periodontal instruments in 432 sites. After the statistical analysis of the data, the BANA Test results showed that in the children 67 (31%) of the sites presented positive reactivity while in the mothers 85 (39,3%) and in relation to the negativity of the BANA test, it was 149 (69%) e 131 (60,7%) respectivilly, this showed a statistically significant relationship between mother's and son BANA variable. The results of the present study let us to suggest that the presence of bacterial periodontophatogens in the children is related with the presence of the same bacterias in their mothers, and that a possible relationship of the periodontal alterations in the mothers can be an important factor in the establishement of periodontal pathologies in children

ABSTRACT
